Do each of your MSDS's contain the following information?
- Product or chemical identity used on the label. _____
- Manufacturer's name and address. _____
- Chemical and common names of each hazardous ingredient. _____
- Name, address, and phone number for hazard and emergency
information. _____
- Preparation or revision date. _____
- The hazardous chemical's physical and chemical
characteristics, such as vapor pressure and flashpoint. _____
- Physical hazards, including the potential for fire,
explosion, and reactivity. _____
- Known health Hazards. _____
- OSHA permissible exposure limit (PEL), ACGIH* threshold
limit value (TLV) or other exposure limits. _____
- Emergency and first-aid procedures. _____
- Whether OSHA, NTP** or IARC*** lists the ingredient as a
carcinogen. _____
- Precautions for safe handling and use. _____
- Control measures such as engineering controls, work
practices, hygienic practices or personal protective equipment required.

- Primary routes of entry. _____
- Procedures for spills, leaks, and clean-up. _____
- American Conference of Governmental Industrial Hygienists.
- National Toxicology Program.
- International Agency for Research on Cancer.
