CONFINED SPACE ENTRY PROGRAM STANDARD-1910.146 DOCUMENTATION REQUIREMENTS

a written permit-required confined space entry program shall be developed and implemented and shall include at least: (d)
  • measures necessary to prevent unauthorized entry.
  • (d)(1)
  • means to identify and evaluate permit space hazards before employees enter.
  • (d)(2)
  • means, procedures and practices for safe entry operations, including, but not limited to:
  • (d)(3)
  • specifying acceptable entry conditions;
  • (d)(3)(i)
  • isolating the permit space;
  • (d)(3)(ii)
  • purging, inerting, flushing or ventilating the space as necessary to eliminate or control atmospheric hazards;
  • (d)(3)(iii)
  • providing pedestrian, vehicle or other barriers as necessary to protect entrants from external hazards; and
  • (d)(3)(iv)
  • verifying conditions in the permit space are acceptable for entry throughout the duration of an authorized entry.
  • (d)(3)(v)
  • means to monitor the space while the job is in progress
  • (d)(5)
  • designate persons who are to have active roles in entry operations and identify the duties of such persons
  • (d)(8)
  • develop and implement procedures for summoning rescue and emergency services
  • (d)(9)
  • develop and implement a system for the preparation, issuance, use and cancellation of entry permits
  • (d)(10)
  • develop and implement procedures to coordinate entry operations when employees from more than one employer are working simultaneously as authorized entrants
  • (d)(11)
  • develop procedures for concluding the entry after operations are completed
  • (d)(12)
  • Review the program using cancelled permits and revise within one year after each entry or as necessary
  • (d)(14)
